Anyone who identifies or becomes aware of suspected or actual cases of malpractice or maladministration must immediately notify QA by completing a Malpractice and Maladministration Report Form and emailing this and any supporting evidence to: malpractice@qualsafeawards.org
2. Handling Whistleblowing Reports:
- Ensure that all reports received are taken seriously, and every effort is made to address them promptly and effectively.
- Designate a specific department or individual responsible for managing whistleblowing reports.
- Clearly define the responsibilities and authority of the designated person to investigate and resolve reported incidents.
